Dear Students,
Students who will be taking the Fall Semester Make-up Exam for the 2026–2027 Academic Year must complete the attached application form in full.
If your excuse is health-related, your medical report must cover at least 3 (three) days, and it must be submitted in person to our department no later than 3 (three) days after the report’s end date. If you are unable to submit the documents in person, you may email them with a wet signature to tdeblm1@gmail.com.
You must submit your application and medical reports to the department secretary’s office by the end of business hours on Wednesday, November 25, 2026.
